GUI front end, take user input output vbs file

Hi, I need a GUI frontend that will work with a VBS scrip that I have. The VBS script has variables that need to be dynamically populated from user input. So the GUI will have a few tabs at the top to allow for different options and text/drop down boxes, select boxes, help (when hovering over any option help will be displayed in that box). It will generate the full vbs script at the click of a button and save it to file on hard drive. SO the end result must be an exe that the user can run, input the required options and then a VBS file is generated. MUST be modular in nature to allow me, later on, to keep adding tabs and new input fields as needed. Source code must be available and documented. I have attached an example of what the GUI could look like but that is something very simple. I want something that looks and feels a lot better than that. As I have said before, apart from the current functionality required, must be modular enough that will allow me to add new tabs, new inputs as I further develop my VB script - I don’t have programming experience so the source code needs to be documented pretty well so I can pick how to add the new tabs, input boxes/buttons, new VBS script as it evolves over time. It is not hard, very simple project as I already have the VBS script all worked out, I can show example of the fields that need to be filled by user input
