Create a PHP based Poll/Survey Script for me

No LATER Change is Allowed*** Hi I need a PHP based Poll/Survey script that will have features like secured Registration with email, username, password and email verification, login, forgot password, Social Login, Admin Panel to have sections for site logo upload, favicon upload, site name, site email, google Recaptcha for login, registration, using the poll, send emails to users, paypal payment, subscription plans, IP blocking. For Poll: Set number of votes a poll can receive (from 0 to unlimited), IP restriction (so that user can vote one per IP), Poll Name, Poll question, poll answer types (like date, email, radio, checkboxes, URL etc), poll time limit, set poll as public/private), poll analytics, show poll results. It looks like many features but actually not. I need full source code and copyright of the full script. I will check this script extensively before releasing milestone. Lowest bidder will win the project. My budget is $45.
