Advanced search from - Search database of users

Hi, I would like a search form that returns user data. I have added extra fields to a user's profile using 'cimy user extra fields'. It creates tables 'wp_1_cimy_uef_fields' with the user id and their answer. it needs to be the same as [login to view URL] However, for this brief, i only need 2 search fields. 1 is a drop down with results from the table (height, hair type etc) and the second is a text field (user names).
