i've a form without any styling & validation. Need to style & a little amendment at the fields. it will take not more than 1 hour.

1) Use some fonts (League Gothic, etc) and coloured boxes for titles (Contact Info, T-Shirts Info, Prints Info, Specialty Services, etc)? box color is #ed1c24.
Field amendment:
2) Put Additional Details field, Date Needed By, and Upload File, into a same box under the title "Notes On Order".
