I have 13 pdfs that need an html form based off of each pdf. The field name in the form must match the field name in the pdf exactly (as well as the ID). The form must be built using html5 and bootstrap styling. The html form should appear much like it's pdf reference (placement, style etc..) but doesn't need to be EXACT. (just whatever makes most sense for the html form first). If you can provide a sample in your bid based off of my pdfs, I would appreciate it. I'll attached the most difficult form with the most fields out of the 13 I'll need.
