The attached “form” needs to put into a PDF (Adobe) document that can be filled in by anyone electronically. The attached “template” needs to be on the top of the page. Information must be organized accordingly: Case information in one section. Clinical Information in one section. Registration information in one section etc…… Please be very organized and CREATIVE (match colors to the Healthpoint Logo) **I don’t need drop down boxes, all fill in boxes must be left blank**
