Building a diagnostic form on a WordPress site

I would like to have a form/app developed that business owners can complete which helps them diagnose whether they are running their business properly. The form will sit on a WordPress site, the user will need to login to the form and each quarter be reminded by email to update their responses. Dependent on their responses wil trigger the system to identify actions they need to take which are then compiled into an email for them to work on during the next quarter. I need to be able to be shown how to amend the questions and responses. The key to this project is keeping it simple. I then want to build a community around it and then get crowd funding for a more advanced version which will then pay for more development time. I see this as a long term project. I have already tested the concept and so know it works. The goal of this project is to get an engaged community
