populate forms with info from relational database

Our site is in asp.net using c#. We have 5 forms that use a relational database to store information. When a user clicks on 'Save' the info is stored to the database in a relational table with one row per input field. I need these forms to be dynamically populated with any existing data. So if a user opens up form with id=5, the page will fetch all data from the database where form id=5, and put the values into the existing fields. Most of the fields are text fields, but there are also some check boxes, or drop downs. If you are interested in this project please let me know and I can show you a test URL of the forms.
