HTML 5 CSS Styling and mysql database

I have two fixes to a website as seen in the attached images. First I need functionality to create a new page that records documents to create a "preferred user" profile to record take an email address and record first & last name, email address, phone number and date of birth from a user. There should be a backend included to display data for the company. Lock at "Part I" below for more information. Part I Look at image "[login to view URL]". The user should type their email and then it is posted to the database. When it does this, it should ask for additional information to include first & last name, phone number and date of birth from a user and record it to a database. There should be a front end for the manager to display data to a page for all users with a birthday for a given month. It should also also have basic CRUD capabilities for the company to manually create, read, update and delete user information from the database. Part II Make the boxes in [login to view URL] and change the white font to match the other color, and make all three boxes the same length when display width is full screen, and make sure the other displays look good. This needs to be done in the next 24 hours. If you cannot, complete this in two days, please do not submit a proposal.
