We need a shopify designer/developer to make a form page on our website which will collect basic validated information (First name, last name, mobile number (otp validation on same screen), email address (email validation), address and a 16 digit unique number (product code) which will be validated through a database where 2000 of such product code would be uploaded for validation. The URL which are using to capture basic details now is [login to view URL] (This URL currently doesn't have otp validation, email validation and also product unique code validation.)
