PHP Contact Form - Very Urgent

Hi, I need a PHP Contact Script made urgently, the current contact form/script was made on a windows server (ASP) but the website is now hosted on Linux so i need them to work on a PHP Script. The current forms can be found here: <[login to view URL]> and <[login to view URL]> I am guessing the form would need to be remade to suit the PHP Script also, however the form layout needs to look the same. The form also has to arrive in HTML Format and when you click Reply in the email it needs to put the customers email in the "To" field. I would also like the layout of the email to look nice also. Bidders MUST show examples of work.
