Fix a form to submit article to K2

I have created a form using Chronoforms that allows users to submit a location which creates a K2 article. The form uses Google maps to allow the user to select the specific location and then populates the form with the correct longitude and latitude. I had this form up and running on my old Joomla 1.5 site using the old Chronoforms v3, but after upgrading to Joomla 2.5 and switching over to Chronoforms v4 I'm unable to get the custom php code to process and combine my elements into the article.
