A Small Android and iOS application to submit two forms

Hi, I need a very small application from which need to submit two forms to a mysql db of a website PLEASE PROVIDE ACTUAL QUOTES ONLY! CHANGING QUOTE AFTER CHATTING IS NOT ALLOWED. PLEASE DO NOT WASTE TIME TO INITIATE DISCUSSION BY UNDER QUOTING. MY REQUIREMENTS ARE GIVEN BELOW AND THATS ALL REQUIRED FOR A QUOTE. 1. Login information should be taken from the online db 2. Once login, they will have two forms, one for a report submission and other for leave application 3. While Submitting the form, location has to tracked and added to the db. 4. Every one hour, current location has to be stored to the db 5. Good Standard UI is mandatory 6. Should provide non-encrypted source code of both android and ios I need bot android and ios versions of this app - to be done separately (not by using anglular or xamarin...).
