stuck on writing java loop logic

Please take a look at the attachments before replying. I have included a screenshot Screen Shot '2020-02-28' which shows 9 data fields ( Opt Out Mail, Opt Out Mail, Email Opt Out, Opt out Phone, Opt Out Fax THESE ARE BOOLEAN logics so ( true/false) and Date Mail, Date Email, Date Phone, Date Fax ( Date type) that appears on my Salesforce FORM from the end user perspective. The API Names ( actual java code that does the business logic in the backend) are as follow starts with A_OptOutEmail, for example. I have all of the code completed for the Date variables [login to view URL] ,but I am stuck on the boolean logics that is the one with the checkboxes in front.... Anytime I checked( Opt out mail, Opt Out Email, Opt Out Phone, or Opt Out Fax ) ( these are the 5 boolean logic), it will pass that information, so that I can use this to trigger an API call from a Zoho form I created. When I check one of these boolean variables, it will show up on my Zoho form as checked or unchecked through a REST API integration which I will do. I am just stuck on the boolean logic for the code.
