eBook Layout/ Graphics and Cover Design

I have a completed eBook and I need someone to assist with the book layout and design that will be used for both eBook and printed formats. I am saying only one design and layout but it will be used for both formats. My book intro is provided below so that you can understand the topic and genre. I am looking for formatting and design. Would include at least one photo on each page, you would need to come up with the cover design and all photos. You will also need to do a table of contents. The book is about 13 pages with text only and about 4,500 words. Once Completed, I would like the book to be between 14-20 pages plus cover and table of contents. The finished product needs to be supplied to me in the form of a Microsoft Publisher 2010 file format. Don’t want the cover to look too scary or intimidating and the entire book should look as professional as possible. The booklet is for an architectural/ design firms prospective clients. The target audience is for preppers/ survivalist with high incomes. Intro: This booklet has been created for those who are ready to begin that process of designing and building a secure home and possibly a shelter. You want to be able to protect your family and yourself against: natural disasters such as earthquakes, flooding, storms, & tornados; criminal acts such as break-ins and robberies; social unrest; nuclear attack/ radiation; chemical attack; EMP; and other TEOTWAWKI type events (The end of the world as we know it.). This booklet is a mini primer on what you should consider along with some suggested solutions in regards to beginning the design of a Highly Secure Home and Underground Bunker/ Fallout Shelter and working with a Professional Building Designer (PBD).
