Hi, Here is what we need: - make fields in PDF as fillable (see attached source pdf) - replace signature fields with text fields. - give each field(including checkboxes) a unique name within the form(we will provide you a convention to follow in naming field) Note: this is a sample task. If you can meet our expectation, you will be our data entry person going forward(500 forms). Attached is a form for this gig (in spanish). We have a sample form that is converted as per our needs in English language(found here: [login to view URL]). So, match to this. Except, for each field name, prefix it by sp_ For example a field named: lic702_student_fullname gets prefixed with: sp_lic702_student_fullname
