web base form submission

Hi This is related to a project i recently posted. I have the web form ready. Requires: 1) A neat welcome page like <[login to view URL]> but much simpler; only copy the menu bar, the message of the day box, resources and sections as well as collections by topics and Recent BigAdmin Features. The title headings will be different. Will advise you. On the right side, a section for customer feedback, list 4-5 countries in a pull down menu and the customer will select the country where his/her project is and press go, a page loads for him/her to enter username and pwd. next loads a page of projects for that country. he /she selects the project and then loads the customer survey form i have with the project name. so here you may edit the form with javascript? Note: For all paegs, follow the color codes of <[login to view URL]> . 2) For now to make things simple, just generic log in (with std id and password) 3) Once logged in, a page loads with 4-5 countries shown. The customer will select the country the form will be presented and customer will fill it in and submit it. A page to see form successfully or fail submitted is needed. To send to emails. 4) When the form is submitted via email to me, what will i receive? a txt file? answers format?
