I want to find someone who is good at providing advice and assistance on putting together a simple 5-10 question survey via Survey Monkey via their "free" offering they have.

I would want the person to be able to speak clear English and have an ongoing consultation over the phone to get the survey set up with the right questions.
This work can lead to more work as the requirements will change over time for a small/medium sized business.
PLEASE you must have done multiple Survey Monkey projects for clients and have an in-depth knowledge on survey methodology and question formatting that encourages responses, not discourage responses.
In addition to helping put together the questions, the successful candidate will then need to be able to build and create a fully working survey based on the discussion/consultation as part of the project.
