I'd like to have a simple GUI app made for Mac OS (Sierra to Mojave compatible) that lets a user enter their Name, Title, Email Address, Phone Number(s), and an optional Quip or Notation and then upon clicking submit creates an HTML mac mail signature that follows a specific template.

Template will be provided by me and it should be accessible to edit at a later date.
