Upgrade vBulletin from 4.1.12 to 4.2.2

Need an upgrade from vB [url removed, login to view] to 4.2.2.
My site has costumized review-sections and the must be working in 4.2.2 when the upgrade is done. Documentation will be provided.
My skin is costumized and will not work as it is with 4.2.2, and this must be fixed as well.
I'm using some plugins from Dragonbyte Tech - most importantly Second Navbar. DBT has recently upgraded all their plugins to work with vB 4.2.2 and everything regarding their plugins must work seamlessly when upgrade is done.
My site was hacked some time ago, and most of the damage is cleaned up, but there is a [url removed, login to view] virus I canb't get rid of - and this must be fixed as well.
