Flash personality quiz

I need someone to build a very simple flash personality quiz. Loads dynamically, blue and white colors, fills a full screen.

The quiz is my own at [url removed, login to view] ? 12 questions in total.

The results would?

A) Tell the user their classroom management style - either

Authoritarian

Authoritative

Laissez Faire

Indifferent

- with a brief description I"ll provide (as on the quiz site).

B)? Ask for an email address to notify of a future site launch at the end.

I have the Classroom Management Quiz in word .doc? with scoring....
