Magento xtento order export - XSL Template

I need an XSL template creating for Magento xtento order export extension.
It needs to be in CSV format:
1) Text delimiter: “ (double quote)
2) Field delimiter: , (comma)
3) Record (line) delimiter: CRLF (ASCII chars 13 & 10)
One line per order line. Fields as follows:
1) Unique ID of order.
2) Account Code (I think this is always “ZZZWEBPAYP”)
3) Order date (DD/MM/YYYY)
4) [blank] (no idea why this is the case)
5) Delivery name
6) Delivery address1
7) Delivery address2
8) Delivery address3
9) Delivery postcode
10) Line quantity ordered
11) Product code
12) Product description
13) Unit price
15) VAT code (1 for 20%, 0 for 0%)
17) Payment name
18) Payment address1
19) Payment address2
20) Payment address3
21) Payment postcode
22) Line number of the current order (not the line position in the export file)
23) Payment method
24) Shipping method
25) Shipping value
26) Notes
27) Coupon value
28) Coupon code
