Create a sample print job using QR codes and labels

Upgrading an existing application. I need some assistance to write some code immediately.
Requirements:
.net
C# / Forms Application
iTextSharp (for generating the PDF to print)
[login to view URL] (or other QR code framework)
Application will be configurable for different label sizes and orientations. I will supply the sample data, you will construct the PDF based on the label size and count requirements. Examples of labels (not limited to) include:
Avery 18663
Avery 8363
Label will look like the following:
