Hello,
I need a simple label designed and have minimal requirements. Please see them below:

• Size: 4" x 6"
• Text with checkboxes next to each word: Sativa, Indica, Hybrid
• Text with straight line beside or underneath it: 'Strain:' 'Date:' 'Weight:'
• Please include some simple/modern design, doesn't have to be elaborate.

Hi!
These are my entries for "Simple Label Design" contest.

The colors are variation of dark blue, dark green, and light blue.
It has a simple pattern.
All shapes and pattern are modern design.
