Contest Brief

I am an author who speaks at schools. After the presentation, we send an order form home with the kids so that they can order books. My current order form is serviceable but doesn't look as professional as I'd like. I need someone to think through the experience and make it look better. I'll need it in an editable form by the end so that I can customize it and put in the name of each individual school.
I've attached my current order form and all relevant graphics files. Please feel free to ask any questions.
*You may use color, but it needs to still look good in black and white.

      Probably .docx would be the easiest, as multiple people will need to be able to change things if necessary.
