I have a quick simple set of modifications for a WordPress website. Modifications / Add-ons for [login to view URL] 1. Create payment options section above the footer on every page with the following text: "For payments we accept all Major Insurance Carriers, Visa, MasterCard, American Express, Discover, Cash and Checks." 2. Replace the current registration form ([login to view URL]) on the website with the new one - replace downloadable file (document name: [login to view URL]). 3. Add new doctor on about page with available details ([login to view URL]) - will go bellow the current doctor. Use the same sort of design language existing on the website. 4. Add the new doctor on the home page also using the details provided. Use the same sort of design language existing on the website. 5. Add "In-Network with all major PPO Insurances" text (at the top of all pages below the very top header with phone number) 6. Add testimonials rotator to the bottom of the homepage. Testimonials from this rotator / slider will contain just excerpt of the full testimonial. When clicked on "read all" it will take you to an a anchor of that testimonial on a new "Testimonials" page.(testimonials are provided - [login to view URL] [login to view URL], [login to view URL], [login to view URL]) 7. Create a "Testimonials" page with all testimonials - testimonials will be laid out on that page horizontally - (testimonials are provided - [login to view URL] [login to view URL], [login to view URL], [login to view URL]) 8. Fix the map on the contact us page by using the new address: 3215 Willowcreek Rd, Portage, IN 46368, USA
