Need Tanning Salon Software Developed

Need software developed for Tanning Salon. I want it to be very clean looking with a white background and spot at the top for the salon logo, which I could import myself. I would like quotes for developing a software cd based on ACCESS or some similar database technology and/OR a web-based database where I could input the following: * Main Screen with Add New Customer, a field to input Customer # and a button to search by Customer #, a field to enter Last Name, First Name and a button to search by last name, first name. * Customer Info Screen- which would be customer first name, last name, address, phone #, initial sign up date, last visit and customer id. Also package or visits bought and the ability to deduct 1 visit every time they come in * Package Screen- This is where all of the tanning packages are listed and you could choose a particular package and add it to a customer screen. This screen will have packages as well as individual visits. * Sales Screen- Where you could pull up the sales for the day as well as enter another date or range of dates to pull up sales. * This database needs to have the capability of merging with a label program to print out the Customer Name and address for promotional mailings for the salon.
