VB6 form to import data from Excel to SQL

Require a form (attached in project file as .frm file) to import data from excel file (attached) into a SQL table. The import process should not use bulk import or bulk copy. I require that the .frm (attached) is completed with the neccessary code to import the data from the EXCEL document into SQL server (table script attached). I have attached the following files from the VB project (applicationevent, the form (.frm) and the main event. I would like the form to produce a popup box if it attempts to load the incorrect file (i.e. the columns in the excel file attached should be the case all the time). If any columns exist that are outside of these columns when attempting to import the file display a message box (this would be before the SQL query runs). The SQL query within the form should use a simple INSERT to insert the data into the NPCMA_Positions table.
