URGENT NURSING

Length: 750 - 1000 words Task: Essay: A critical analysis of comprehensive social and health history data for two older persons, one older person who considers she or he is ‘healthy’ for this stage of their life and for their age, and a second older person who considers they are experiencing ill health and/or dysfunction. * Critically analyse the nursing assessment data from the comprehensive assessments undertaken, make nursing statements or nursing diagnoses (not medical diagnoses) of the data in relation to identified actual and 'potential or at risk' health problems, which will demonstrate differences in these two persons’ social and health histories. Undertake comprehensive assessments of two older persons, one who perceives that they are healthy for their age and one who perceives they are in a state of ill-health or dysfunction at this stage of their life and age (attach these assessment tools (FHP & additonal tools) as appendices with your essay). NOTE: You have been provided with a long version of the functional health pattern (FHP) tool for comprehensive assessment in session 1. This FHP tool may indicate problems in one functional pattern that may require further assessment/s to be undertaken with an additional tool (ie if the patient is cognitively impaired (has delirium, dementia, or depression - use an MMSE, CAM or Cornell or RUDAS tools as appropriate). Or the older person may be malnourished and you may then need to choose to use a Mini Nutritional Assessment tool (MNA); or a fluid and electrolyte tool; or a urinary incontinence tool; or a mobility or skin assessment tool etc. These tools provide you with data from a comprehensive assessment, which you can print off and have with you to collect data during the interview...if you feel it is required for your 'data collection for your essay preparation'. * compare the similarities and differences in the chosen health problems (nursing diagnoses, not medical diagnoses) between the two older persons and how these health problems impact on their quality of life and lifestyle. The information will then need to be analysed (diagnosed) in terms of what are the nursing problem/s or potential problem/s in one or more of the FHP areas. You need to write up the actual problems using a 3 part statement and a 2 part statement for a probable nursing problem. Firstly, critically analyse the two sets of data for the two older people (for a guide to the analysis process refer to section 1 nursing process, nursing diagnosis and evidence based nursing by Ackley & Ladwig, , pp. 1-14, 2008. This chapter also provides an example of analysing data using a case study by: 1. highlighting or underlining the relevant symptoms (defining characteristics/evidence for the problem) 2. making a short list of the symptoms; clustering similar symptoms; analysing/interpreting the symptoms, 3. then select a 3 part statement for the nursing diagnosis label 4. ie the name of a problem that fits with the appropriate related factors and the defining characteristics. This data collection from varied tools, is only 'preparation for your essay', as you will need to critically analyse all this data in order to decide which information you will use to develop your essay. You can present your analysis in tables, flow charts, figures, graphs. This information in tables etc will not count as word limit & you will need to place them in your essay near where you are discussing the content eg "(see Table 1)" etc. You can then select what you will discuss as you do not have to discuss all the data...only what you feel will be useful in comparing between the two older people. • Following your critical analysis of the two sets of data using the process given above, or the data collected from any other assessments you have undertaken, you now need to 'write your essay'.
