Build a Dynamic Product Label Generator

I need a product label generator which will read a pre-defined PDF template and populate it with relevant information chosen by the user.
The application interface will allow the user to :
1. Select a Model Number from a drop-down menu
2. Select / Input the date of choice.
3. Click "Generate" to create a PDF Label.
The system should look up all the relevant information for the chosen model number from sheets (eg. google sheets) and generate a PDF type product label.
You will be provided with:-
1. Google sheets with all the information about the product.
2. PDF Label Template
