Survey application for desktop and ipad (responsive)

Greetings, I am looking for designer and developer to create complete survey application from start to finish using php and mongo db. The application should be accessable using web browser on desktop or ipad (responsive) no need for mobile phone. Basically it will have following features on high level; more details as we progress through development 1. User login (hardcode in mongo db, no need to create a seperate user administration interface), 3 roles user which responds survey manager which manages complete survey and generates charts and reports administrator which manages the survey, and application specifications 2. Each survey will have Title and Description Category Sub Category Questions ...... (textarea, dropdown, check, radio, attachment) 3. Example workflow, Admin creates survey meeting for microsoft company and then fill basic detaisl like company name etc, then he meets with their it team for category B and its sub category to answer questions, he captures all details similarly he meets other employees to get more information and complete survey then he generates report using phpdocx question bank will have question, sample response, reommendation, score, weight (this score will be calculated the final report score and charts questions can be reorganized in survey template Survey can be created from a question bank, and there can be many survey, which can be responded by company 4. generate few charts and then word report using phpdocx; using word template and paramaters (i have working version of phpdocx) The developer must use a themeforest template will be provided, developer must good with html designing and css.
