SQL query select rename multiple choice in php file

I am looking to rename multiple choice answers client side (php file) that are showing as numbers. Example what I now have in my PHP file: 'sql_query' => "SELECT element_10,element_2,element_3,element_5,element_4,element_6,element_8,element_9 FROM ap_form", Here's what I need element_5 has 2 choices Choice 1 is showing up 1 I would like it to be MA Choice 2 is showing up 2 I would like it to be NH element_9 has 3 choices Choice 1 is showing up 1 I would like it to be <i class="fa fa-check" aria-hidden="true"></i> Choice 2 is showing up 2 I would like it to be <i class="fa fa-times" aria-hidden="true"></i> Choice 2 is showing up 3 I would like it to be <i class="fa fa-clock-o" aria-hidden="true"></i> element_6 is a phone number I would like formatted also it is now a 10 digit phone XXXXXXXXXX, if possible I would like it to format to XXX-XXX-XXXX
