Simple PHP-based Survey for receiving feedback from clients

Survey Login • Login with username and password • Admin panel should keep the admin signed in until logged out. Shouldn’t require sign in multiple times per day Survey Form • Customer fills out form on WordPress page that matches look and feel of Q-PLUS Lab’s website • There are 7 survey questions; 5 which are required for submission and 2 optional • 1 Text area (Comments) which is not required Each submission must contain unique data and not carry-over data from other submissions. (We’ve had this problem with HubSpot forms and surveys). Admin Panel Functions • Admin panel should create unique survey with Job Number and supplied date in URL string o Survey link created should only be able to be used once • Admin panel should show list of surveys created o Admin should be able to delete created surveys from survey list interface and database from within admin panel  * Should be able to delete unwanted surveys such as tests • Admin panel should show status of created surveys (highlight with green background if survey has been filled out) • Ability to export CSV / Excel survey data o Selectable by date-range o Comments should also be exportable Submission Notification • Email notification to admins that surveys have been completed • Q-PLUS will define emails that receive notifications • Q-PLUS will define content that is in email notifications • Ability to edit text in email notification of form completion After the Survey is filled out • Customer redirected to Thank You page • Customer should not be able to submit survey multiple times Additional: Create written instructions for the general use of the application. This should include which files do what and which files can be edited to make changes including the ability to edit any visible text the client will see. After delivery we will setup a session to show the functionality and how to make changes to the PHP files. Available Resources: • We will provide access to the software written in PHP. This could be modified to work with generating a WordPress Form versus the current HubSpot Form. • We can provide access to the PHP code that was previously written that worked on our former Drupal website.
