Revise Wordpress Contact Page

I need someone to revise my contact page on my website. I added a questionnaire that clients will fill out when they reach out to us. That part is fine. However, I would like to add some stagnant details on the side (phone, email address, and social media icons). So we would like to have this done in two columns, the questionnaire on the left and the stagnant contact details on the right. If possible, it would be great for the contact details to stay stationary as they scroll to fill out the questionnaire. On the mobile device, I would like the stagnant details to appear above the questionnaire.
