Looking for a programmer to write some HTML code

I have an existing website and I have a page to request a free quote for printing [login to view URL] . I would like to add into the page where you can add multiple artwork files and it will upload to my ftp, and then receive and email with info request and a link to down load the file. I also have another domain [login to view URL] where clients can upload files and it works through a company called [login to view URL] . It works well but there's no support when there is a problem, they don't answer email. I would like to create that type of application so I have control, right now my clients are uploading files but I don't get them.
