Enhance Adobe PDF Form for Medical Practice

I'm looking for someone that can enhance the forms I have already created with scripts that (1) transfer data from one page to anohter (2) do calculations I will need the first three pages, then the third page 10 more times, but many of the fields must be different so they are not erased when new data is put in. I am looking for some data to cary through to every page: patient name and height. Then I'm looking for some data to be pulled from the previous page: previous dx, previous wt, previous BMI, previous waist circ. It would be ideal if it would pull from the last entry and not just the last page, as sometimes there isn't a new weight taken. Also, the previous dx is a check box system, but if a box is checked I want it to appear in the text box on the subsequent pages as the written words next to the check box. Please read and review the document. I'm looking for a fast turnaround. This should be easy for someone with Adobe skill.
