Database to Replace Excel Tracking - Change Management

Hi there, I am looking to have a web based application developed for tracking change management in a system. Currently I am looking to get a working prototype together as a demonstration of concept, what I mean by this is I primarily want to focus on the process and concept and have less concerns about how it looks. Once I have a working prototype we can make it look appealing after. The system will be using a database with a web front end to replace the current system which is a combination of filling out word documents and tracking the documents in an excel spreadsheet. I would like the parts of the document to be easily configurable. Primarily it will be viewed using a web browser on a desktop PC. My preference would be to have the prototype site developed in PHP with a MySQL database however I am open to recommendations that would make the development easier / more streamlined.
