customize web application

web application. we want to dynamically add fields and database column. The list of items in the field type are. Text Field, Fixed Narration, Master List, Date, Time, Numbers. The nature of the field react according to the type we select. Default will be Text. If you refer to the above pic after field length one more field required field style. Field Style is contain 4 items i.e, Mandatory, Normal, Hidden, Read Only. The style we select the field has to be react accordingly. Default will be Normal Once we complete the above process and click on Ok it will save and add the No. Of fields given in the grid. otherwise cancel means close and exit from the screen.
