Build Templates with LibreOffice or CSS Grid

We make the original setup of the templates with LibreOffice and You must make the software to automate making the templates with our .odt files. This can also be done with the help of LibreOffice or CSS Grid, etc. (to be discussed).
This project has 3 steps:
a) Make a template from a LibreOffice document, made by us, with .odt extension and change the frames in that document into fillable fields. (See attached document as an example - You must be able to make a template with this small file, before hand, to demonstrate Your ability to do this).
b) Make another template for an invoice (we supply the original odt file) You add all formulations (additions, subtractions, multiplications, divisions) included in the template.
c) Add a MySQL Data Base for storing all information needed.
N.B.: A template is like a pattern used for things that repeat themselves in every document. Only the fillable fields can be changed.
