Calendar Iphone Application for Recording Moods & Health Stats

I need someone to Design and develop an iPhone app. The app should track general mood- Good or Bad- and then in depth mood ie depressed, anxious, happy, elated, fatigued etc, they should also be able to write detailed notes for that day if they so please. These should be recorded on a calendar and there should also be a spot where the user can find stats on their changes in mood.
In addition to the above I want there to be a feed page with predetermined groups where people can chat amongst each other like bipolar, depressed, etc.
If possible I would also like it to allow people to login via facebook or instagram, but it's not absolutely necessary at this stage.
When writing a proposal please include all of this. I don't want any surprises in regards to how much I will pay.
