XenServer reattaching "failed" LVM storage

We have a Citrix XenServer 6.5. After a reboot the SR was not attached. We tried the standard methods for reattaching the SR, but it did not work.

At xe pbd-plug uuid=ac46c3ad-510b-f18d-a3bb-7af6fd8d47f6, we only get:
