need a form for my APP

I developed an app for myself with appmachine.com. See attached Appstore logo … I’m missing a good form registration for played scores. I have in appmachine the possibility to import javascript code / html files. I would like to have a form like this: [login to view URL] With 2 and 3 dependant dropdown fields. De data I deliver is in jSon format: [login to view URL] The first dropdown should be a list of all the courses distinct (field = course), when selecting a course the second dropdown (field = name) should update and select all the tee’s for that course with ID (field = calculation). I also need a hidden field (user emailadres) where I should give you the value when calling the form. And last but not least 18 input fields, Hole 1 to Hole 18, only numbers input from 1 to 10.
