Backend + frontend systems for automating health clinic workflow

Hello Freelancers! The project is relatively straightforward. I have a clinic with a need to switch to a paperless workflow, the following list sums up most of what is needed to be designed within the new system you will propose: The first part is the filing system; Patients and staff will fill these out electronically, can be through a web page or app on a tablet (meaning all platforms). 1 - Identity - name height weight number email etc. 2 - patient consent form(s) - digital signature (meaning they will sign using a stylus) 3 - family medical history - drop down menus most likely 4 - pain location / chart - drop down menus most likely Staff 1 - patient questionnaire - text fields 2 - patient medical history - text fields 3 - patient record -treatment plan - text fields 4 - patient record - imaging - text fields + images 5 - patient record - progress notes - text fields 6 - patient invoicing - automated and linked POS - printable invoice This is roughly what is required from the system. Other requirements: 1- Ideally you would propose which platform and language you would utilize. For example you could decide this entire system can utilize some sort of database driven website, but of course that would mean I would have to utilize the internet to have my records, if you do propose something like this kindly provide me a way to have a local copy at all times. 2- The language or platform you choose to propose this in should allow me to expand upon it easily in the future, kindly refrain from proposing something in an obscure language that would restrict my choices in the future to only you or others familiar with this type of platform / code. It would definitely hurt future business between us. 3 - I have plans to integrate this systems with other services that utilize data analytics, so a way to convert the compiled data into common formats for non-tech savvy users would be ideal, an example would be export to Excel or the like. The second part is the user and attendance system (which should work together with the filing system). So I would need different levels of permissions and authority (e.g. admin doctor nurse patient etc.) The user database will allow me to do interesting things with patients in the future as well as contribute to the patient record and filing systems. But for the purpose of this proposal I need an attendance system that would allow me to keep track of staff, this would ideally be triggered bio metrically through a fingerprint sensor but I'm open to suggestions.
