Adobe Acrobat Forms Ver 9 Creation

We need to create a number of Adobe Acrobat Forms in Version 9 We have source documents in diffent programs, i.e. Word, excel, Publisher. We need these converting into forms in Adobe Acrobat 9. You will ned to reference the source document and recreate in Adobe Acrobat. We dont want them just copied. They will include maths calcualtions, rule checking, options and drop down lists. We want to find a developer that will produce the first form and then continue to work with us for future forms. Quoting for each as the requirement arrises. These forms have to be good quality and professional. Attached is an example. This needs to be made into a form. The user needs to select the type of membership they want from a list, and the date the membership should start. They also need to tick if they are an exsting memebr. IT will then calculate the fees and add on the joining fee if they are a new member. It needs a discount option where they can enter a code "we give them" and it will reduce by either a percentage or by a fixed amount and display the discount on the form. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Adobe Acrobat version 9
