3D Model of Men's Dress Shirt - Repost

I need a Photo realistic 3D Model of a men's dress shirt along with rendering of texture. The 3D model for photoshop needs to be filled with different fill patterns. I need just one standard version of the shirt (no different cuffs, styles etc.). Only one fabric pattern will be applied to the shirt at once (no different pattern for cuff, for instance). The job is completed if all provided test pattern can be filled in to this model in a easy way in photoshop and a satisfactory high quality result can be reached.
