Fill in spreadsheet with medical data from Word document (recurring job)

This recurring job requires the data entry of text from a Word document into an Excel spreadsheet. There is not a lot of text to enter, but accuracy and proper formatting are very important. You will be provided with a Word document that contains text and screenshots which I would like transcribed into an Excel spreadsheet. The screenshots are from data tables in medical books. The data should be transcribed with the formatting described below. In addition to these instructions, I have attached a sample Word document (what I will send you) and sample Excel spreadsheet (end product) which should make the job clear. The first job will be a trial to see if we work well together. After that, there will be much more work. I can do a fixed price per article or an hourly rate, depending on how fast you work.
