Create Desktop Application to get data from fingerprint device and upload it to MySQL database

I want a desktop application to get data from multiple fingerprint devices and upload the data to MySQL database.
Requirements:
1. C# Application.
2. Clean and well documented code.
3. A form for adding MySQL database settings [host, username, password, database name].
4. A form for machines configuration [ips, ports, sensor_ids, passwords].
5. The application should run in the background and when windows starts.
6. A shortcut to get the application to the screen [C+R].
7. A timer configuration, to set how often the application should upload the data [Ex: every 5 minutes].
8. The data uploading should be incremental [not replace].
9. A progress bar to show the percentage of uploading.
10. A button to upload data manually.
11. Display error message with failed devices [IPs].
12. Display count down for next upload based on timer configuration.
13. A form to test data of specific device, with [ip, port, sensor id, password] and table to show the data when retrieved from the device.
14. The application should have installer.
15. Source code to be delivered.
