Phone Inquiry Sales Lead Customer Database for Android Tablet

We currently receive telephone calls inquiring about products we sell locally. We currently hand write the leads onto an excel spreadsheet and cross them out after the sale is complete. The current input fields include name, state, received date, item of interest, telephone number and notes. We would like to make an android app that allows us to key in the information into a tablet. With requirements as follows MAIN SCREEN: - New Lead - Open Lead Summary - Edit Lead - Closed Leads - Reports - Export NEW LEAD: - Simple lead input screen with first name, state, item, phone number, notes, open/closed lead, pickup date/time, price and date field OPEN LEAD SUMMARY: - Show summary of all open leads including first name, phone number, item, cost, state, pickup time/date and notes - sortable by date, state or phone number. - Clickable lead open to 'edit lead' screen EDIT LEAD: - Display same as 'new lead' screen with all fields filled. Ability to edit all and close lead by changing to closed lead CLOSED LEADS: - Simple lead input screen with first name, state, item, phone number, notes, open/closed lead, pickup date/time, price and date field - Filterable by date or state or phone number - Editable including reopening lead by selecting open on screen. REPORTS: - Saving records by phone number. - search customer by name or phone number to show report of all sales - Sortable by price, date, state, or phone number. - Include total price on bottom and filterable by date EXPORT: - exportable database of customers by phone number that includes all info. CSV ot TXT output. Customer database should be able to list sales/dates/price for returning customers. Simple is better than extravagant. Large font for easy reading outside or inside. Looking to spend $150 or less on this project. MUST BE ABLE TO COMMUNICATE DURING DAYTIME HOURS NY EST USA AND HAVE GOOD ENGLISH SPEAKING/COMMUNICATION.
