Create Radio Buttons On Opt-In Form To Change Subscription List

Hello, I've attached a php page where the work will occur. I need you to ad a radio button set for each option... the radio buttons will ALL be empty upon page load, and it is NOT mandatory to select one. But if a radio button IS selected, it will modify 4 values in the form element. Here are the 4 values it will modify: 1. name="meta_web_form_id" value="637047658" 2. name="unit" value="catfish_nxt" 3. name="redirect" value="/redirect/[login to view URL]" 4. name="meta_redirect_onlist" value="/redirect/[login to view URL]" Then when they enteir their name and email address, depending on what radio button is selected (if any) those values will bei n there for the form submit. Also, I'll need a place to set "default" values for if no radio button is selected... This should work without any problems from IE 6 through IE 8, and on the similar later versions of firefox and other popular browsers. Pretty simple.
