icon/something which clearly shows it's for rearranging the list items. When a user tap on it, then it should go to a screen where user can reorder/hide the list items.

When user tap on one of the list items ( patient), show how patient screen looks ( existing screen without tab)

Looks good. Can you also show grouping of the list items ?. Basically we need separate it into the following group similar to the attached new screen in my orginal description.
And also, can you show how to reorder/hide the the items ?
